That day, an IATF Auditor's Single Sentence Turned Our Team Leader's Face Green
Do you remember that IATF audit three years ago? At the time, the CPK report for our new process had just come out. The auditor was calm, but when he saw the DPMO value for a major customer, he suddenly looked up at our team leader and asked, "What is your corrective action for this DPMO of 6210?" The team leader's face instantly went pale because that data simply wasn't prepared. That's when I profoundly realized that an audit isn't a show; you really need to deliver.
Audit Nonconformity? It's Not Fault-Finding, It's Finding the Cure
To be frank, when an IATF audit issues a nonconformity, especially a "major nonconformity," its purpose isn't to create trouble for you. It's to make you identify the root cause of the problem and resolve it thoroughly. Imagine your product has an issue, and the customer is complaining. You can't just say, "We know there's a problem," right? You need to provide an explanation, tell them what you've done, and how you'll ensure it won't happen again next time. This is the spirit of corrective action.
So, the key is, when you're issued a major nonconformity, the most important thing is "how to address it," not "how to explain it." No matter how much you explain, the problem remains unsolved.
How to Do It in Practice? Three Steps to Handle It
When dealing with corrective actions for major nonconformities, we usually follow these three steps:
- Containment & Immediate Action: As soon as the auditor raises an issue, you must immediately find a way to narrow the scope of the problem to prevent it from escalating. For example, in the DPMO 6210 case mentioned earlier, we immediately pulled that batch of products for 100% inspection. Even knowing it would delay shipment, we had to first ensure that no nonconforming products would be released. This is about stopping the bleed.
- Root Cause Analysis: Stopping the bleed isn't enough; you need to find out why it's bleeding. This is when tools like 5 Whys and Ishikawa Diagrams (Fishbone Diagram) must be employed. For instance, when our DPMO surged that time, we later discovered it was due to insufficient calibration frequency of a certain measurement equipment, leading to parameter drift, yet no one on the production line noticed. Frankly, this is the most time-consuming but also the most valuable step, as it unearths the crux of the problem.
- Corrective & Preventive Action (CAPA): After identifying the root cause, you must propose solutions and find ways to prevent future recurrence. For the insufficient equipment calibration frequency issue, we established a new calibration plan, doubling the frequency, and implemented an automated reminder system. Furthermore, we also checked the calibration frequencies of other similar equipment; this is prevention.
In other words, you first need to put out the fire, then find the source of the fire, and finally install sprinklers.
The Most Common Pitfall: Treating the Symptom, Not the Disease
The most outrageous case I encountered was when an audit found that a certain inspection station's SOP was ambiguously written, leading to inconsistent judgment standards across different shifts, resulting in a major nonconformity. Our team leader simply had someone revise the SOP and submitted it. When the auditor saw it, they directly asked, "How do you ensure that the revised SOP will actually be followed by on-site personnel? How do you ensure similar issues won't occur at other stations?" The team leader was dumbfounded on the spot. Frankly, this is a classic example of "treating the symptom, not the disease." Merely revising the SOP without training personnel or reviewing the applicability of other SOPs is simply perfunctory.
A truly effective corrective action must be able to answer: "How do you ensure this specific problem will not recur?" and "How do you ensure similar problems will not occur elsewhere?"

Go back and look at the SOPs you have. Is there any one that you feel might not be clearly written, leading to inconsistent judgment standards among different people? Start with this small area.
